Sample Social Security Award Letter

As we know, for the Medicaid application process Medicaid asks for a lot of documentation. In many states, Medicaid does not have access to the social security system and require the social security award letter. The document below is received at the end of each year for the following year. I am attaching a sample letter.

If you know anyone who may be applying for Medicaid, make sure they save this document. If they don’t have it they can go online to ssa.gov and create an account.
